Title: Clinic Nurse (Adult & Pediatric Clinic) Job Description: Position Overview: As a Clinic Nurse in an Adult & Pediatric Clinic, you will be responsible for providing compassionate and comprehensive nursing care to patients of all ages.
Your role will involve conducting patient assessments, administering treatments, educating patients and families, and collaborating with healthcare providers to ensure the delivery of high-quality care in an outpatient setting.
Key Responsibilities: Patient Assessment and Triage: Perform initial and ongoing assessments of patients across the lifespan, including obtaining vital signs, conducting health screenings, and documenting medical histories.
Triage patients based on the severity of their conditions and the urgency of their needs, facilitating timely access to appropriate care and interventions.
Nursing Interventions and Procedures: Administer medications, vaccines, and treatments as prescribed by healthcare providers, ensuring accuracy, safety, and compliance with established protocols.
Perform nursing procedures such as wound care, injections, and specimen collection, following standard operating procedures and infection control guidelines.
Pediatric and Adult Care: Provide age-appropriate nursing care to pediatric and adult patients, addressing their physical, emotional, and developmental needs.
Assess growth and development milestones in pediatric patients, providing guidance and education to parents and caregivers.
Patient and Family Education: Educate patients and families about their health conditions, treatment plans, and self-care strategies, utilizing clear and understandable language tailored to the patient's age, cultural background, and literacy level.
Offer counseling on preventive healthcare measures, nutrition, and lifestyle modifications to promote overall wellness and disease prevention.
Care Coordination and Collaboration: Collaborate with interdisciplinary team members, including physicians, nurse practitioners, medical assistants, and support staff, to develop and implement individualized care plans for patients.
Coordinate referrals to specialty services, diagnostic testing, and community resources, ensuring continuity of care and comprehensive support for patients' healthcare needs.
Documentation and Communication: Maintain accurate and timely documentation of all patient assessments, interventions, and communications in electronic health records (EHR) or other designated systems.
Communicate effectively with healthcare team members, providing updates, sharing pertinent information, and facilitating smooth transitions of care for patients.
Qualifications: Valid nursing license issued by the state regulatory board.
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) degree preferred.
Certification in Basic Life Support (BLS) required.
Minimum of 1-2 years of nursing experience in a clinic or ambulatory care setting preferred.
Strong clinical assessment and critical thinking skills, with the ability to prioritize and manage patient care in a fast-paced environment.
Excellent interpersonal and communication skills, with a patient-centered approach to care.
Proficiency in computer skills and electronic health record (EHR) systems used for documentation and patient care management.
